Langkah-Langkah Mendirikan Rumah Sakit

Dinas Kesehatan DKI Jakarta mengeluarkan beberapa persyaratan dalam proses mendirikan rumah sakit umum atau rumah sakit khusus di Jakarta. Adapun langkah-langkahnya adalah sebagai berikut:

I. MELENGKAPI :

Membuat surat permohonan Persetujuan Pendirian RS kepada Kepala Dinas Kesehatan, dilampirkan foto copy kelengkapan seperti tersebut di atas (point 2 s/d 7)
1.Proposal Rumah Sakit
2.Akte badan hukum yang di sah kan oleh Departemen Kehakiman dan HAM
3.Sertifikat tanah
4.Pernyataan tanah tidak sengketa
5.Foto copy pembayaran Pajak Bumi dan Bangunan (PBB)
6.Pernyataan persetujuan warga sekitar dan domisili dari lurah setempat
 

II. PROSES DI DINAS TATA RUANG

Pemohon membuat surat permohonan pembuatan Advice Planning (keterangan rencana kota) dan Blok Plan (rencana tata letak bangunan RTLB), kepada Kepala Dinas Tata Ruang Prov. DKI Jakarta dengan melampirkan Rekomendasi dari Dinas Kesehatan dan gambar rencana arsitektur. Tembusan ditujukan kepada:
– Walikota setempat

– Suku Dinas Tata Ruang setempat

Berdasarkan butir II.1 Tim Pengukur situasi tanah (Dinas Tata Ruang; Suku Dinas Tata Ruang; dan penilik kecamatan) melakukan pengukuran.
Berdasarkan butir II.2 Dinas Tata Ruang akan menerbitkan Advice Planning dan Block Plan termasuk KDB (Koefisien Dasar Bangunan) dan Peruntukan.
4. a. Untuk perubahan peruntukan dan KDB, pemohon mengajukan surat ke Gubernur.

    b. Setelah ada Surat Keputusan Gubernur, Blok Plan dan Advice Planning segera akan disahkan oleh Suku Dinas Tata Ruang dan Dinas Tata Ruang.

III. KANTOR TRAMTIB PROV. DKI JAKARTA

  Pengurusan Undang-Undang Gangguan (UUG)

 IV. PEMOHON MEMBUAT STUDI KELAYAKAN

 V. PEMOHON MEMBUAT AMDAL ATAU UKL/UPL

 VI. PROSES DI KANTOR DINAS PENATAAN DAN PENGAWASAN BANGUNAN (P2B)

1. Permohonan untuk mendapatkan Izin Pendahuluan (IP) Membangun Rumah Sakit dengan lampiran :

– Badan Hukum (PT/ Yayasan, dsb)

– Rekomendasi dari Dinas Kesehatan

– Gambar Struktur, Arsitekur dan ME

– RAB

– Advice Planning

– Surat Tanah dan PBB

2. IMB biasanya terbit 2 Bulan setelah IP.

3. Sebelum terbit Izin Pendahuluan (IP), harus dilakukan terlebih dahulu pematokan AS bangunan oleh Suku Dinas Tata Ruang setempat

4. Izin Penggunaan Bangunan (IPB) diterbitkan oleh Dinas P2B.

 VII. PEMOHON MENGAJUKAN PERMOHONAN UNTUK MENDAPATKAN IZIN SEMENTARA RUMAH SAKIT DARI DINAS KESEHATAN PROVINSI DKI JAKARTA

Dalam proses pendirian rumah sakit amat dibutuhkan tim konsultan yang mampu membimbing pemiik rumah sakit dalam proses teersebut mulai tahap perencanaan hingga operasional.
