SASARAN 2 : MENINGKATKAN KOMUNIKASI YANG EFEKTIF

Rumah sakit menetapkan regulasi untuk melaksanakan proses meningkatkan efektivitas komunikasi verbal dan atau komunikasi melalui telpon antar-PPA.

Rumah sakit menetapkan regulasi untuk proses pelaporan hasil pemeriksaaan diagnostik kritis.

Rumah sakit menetapkan dan melaksanakan proses komunikasi “Serah Terima” (hand over).

Komunikasi dianggap efektif bila tepat waktu, akurat, lengkap, tidak mendua (ambiguous), dan diterima oleh penerima informasi yang bertujuan mengurangi kesalahan-kesalahan dan meningkatkan keselamatan pasien.
Komunikasi dapat berbentuk verbal, elektronik, atau tertulis. Komunikasi yang jelek dapat membahayakan pasien. Komunikasi yang rentan terjadi kesalahan adalah saat perintah lisan atau perintah melalui telepon, komunikasi verbal, saat menyampaikan hasil pemeriksaan kritis yang harus disampaikan lewat telpon. Hal ini dapat disebabkan oleh perbedaan aksen dan dialek. Pengucapan juga dapat menyulitkan penerima perintah untuk memahami perintah yang diberikan. Misalnya, nama-nama obat yang rupa dan ucapannya mirip (look alike, sound alike), seperti phenobarbital dan phentobarbital, serta lainnya.
Pelaporan hasil pemeriksaaan diagnostik kritis juga merupakan salah satu isu keselamatan pasien. Pemeriksaan diagnostik kritis termasuk, tetapi tidak terbatas pada

  1. pemeriksaaan laboratorium;
  2. pemeriksaan radiologi;
  3. pemeriksaan kedokterannuklir;
  4. prosedur ultrasonografi;
  5. magnetic resonance imaging;
  6. diagnostiknjantung;
  7. pemeriksaaan diagnostik yang dilakukan di tempat tidur pasien, seperti hasil tanda-tanda vital, portable radiographs, bedside ultrasound, atau transesophagealechocardiograms.
