MONITORING DAN EVALUASI BERKELANJUTAN ANGGOTA STAF MEDIS

Rumah sakit melaksanakan proses yang seragam untuk melaksanakan evaluasi mutu dan keselamatan asuhan pasien yang diberikan oleh setiap anggota staf medis. Penjelasan dari istilah yang dimuat di standar ini sebagai berikut:

Monitoring dan evaluasi berkelanjutan 

Monitoring dan evaluasi adalah proses dengan kegiatan terus menerus mengumpulkan dan menganalisis data serta informasi tentang sikap, perkembangan profesional, dan hasil dari layanan klinis anggota staf medis. Pimpinan medik/unit layanan bersama komite medis bertanggung jawab untuk mengintegrasikan data dan informasi tentang staf medis dan mengambil tindakan bilamana diperlukan. Tindakan segera, dapat dalam bentuk nasehat menempatkan kewenangan tertentu di bawah supervisi, pembatasan kewenangan, atau tindakan lain untuk membatasi risiko terhadap pasien, dan untuk meningkatkan mutu serta keselamatan pasien. Tindakan yang lebih lama adalah menggabungkan data dan informasi menjadi rekomendasi terkait kelanjutan keanggotaan staf medis dan kewenangan klinis. Proses ini berlangsung palingkurang3tahun.Tindakan lain mungkin memberitahu anggota staf medis yang lain tentang sikap dan hasil layanan klinis yang ada buktinya di data dan informasi dari anggota staf medis.

Monitor dan evaluasi berkelanjutan staf medis menghasilkan informasi kritikal dan penting terhadap proses mempertahankan keanggotaan staf medis dan proses pemberian kewenangan klinis. (lihat juga KKS 9 dan KKS 9.2). Walaupun dibutuhkan 3 tahun untuk memperpanjang keanggotaan staf medis dan kewenangan kliniknya, prosesnya dimaksudkan berlangsung sebagai proses berkelanjutan dan dinamis.

Masalah mutu dan insiden keselamatan pasien dapat terjadi jika isu kinerja klinis anggota staf medis tidak dikomunikasikan dan dilakukan tindakan.

Proses monitor dan evaluasi berkelanjutan dimaksudkan untuk

  1. Meningkatkan praktik individual terkait mutu dan asuhan pasien yangaman;
  2. Digunakan sebagai dasar mengurangi variasi di dalam KSM (kelompok staf medis)/unit layanan dengan cara membandingkan antara kolega, penyusunan PPK (panduan praktik klinis), dan clinical pathway; 
  3. Digunakan sebagai dasar memperbaiki kinerja kelompok staf medis/unit layanan dengan cara membandingkan acuan praktik di luar rumah sakit, publikasi riset, dan indikator kinerja klinis nasional bila tersedia.

Monitoring dan evaluasi berkelanjutan dari staf medis memuat 3 (tiga) area umum, yaitu perilaku, pengembangan profesional, dan kinerja klinis.
