PERESEPAN DAN PENYALINAN

Rumah sakit menetapkan staf medis yang kompeten dan berwenang untuk melakukan peresepan/permintaan obat serta instruksi pengobatan. Staf medis dilatih untuk peresepan/permintaan obat dan instruksi pengobatan dengan benar. Peresepan/permintaan obat dan instruksi pengobatan yang tidak benar, tidak terbaca, dan tidak lengkap dapat membahayakan pasien serta menunda kegiatan asuhan pasien. Rumah sakit memiliki regulasi peresepan/permintaan obat serta instruksi pengobatan dengan benar, lengkap, dan terbaca tulisannya.

Rumah sakit menetapkan proses rekonsiliasi obat, yaitu proses membandingkan daftar obat yang dipergunakan oleh pasien sebelum dirawat inap dengan peresepan/permintaan obat dan instruksi pengobatan yang dibuat pertama kali sejak pasien masuk, saat pemindahan pasien antar unit pelayanan (transfer), dan sebelum pasien pulang.

Elemen Penilaian

  • Ada regulasi peresepan/permintaan obat dan instruksi pengobatan secara benar,lengkap,dan terbaca, serta menetapkan staf medis yang kompeten dan berwenang untuk melakukan peresepan/permintaan obat dan instruksi pengobatan.
  • Ada bukti peresepan/permintaan obat dan instruksi pengobatan dilaksanakan oleh staf medis yang kompeten serta berwenang.(D,O,W)
  • Ada bukti pelaksanaan apoteker melakukan rekonsiliasi obat pada saat pasien masuk, pindah unit pelayanan, dan sebelum pulang.(D,W)
  • Rekam medis memuat riwayat penggunaan obat pasien.(D,O)

Untuk menghindari keragaman dan menjaga keselamatan pasien maka rumah sakit menetapkan persyaratan atau elemen penting kelengkapan suatu resep atau permintaan obat dan instruksi pengobatan. Persyaratan atau elemen kelengkapan paling sedikit meliputi

  1. data identitas pasien secara akurat (dengan stiker);
  2. elemen pokok disemua resep atau permintaan obat atau instruksi pengobatan;
  3. kapan diharuskan menggunakan nama dagang ataugenerik;
  4. kapan diperlukan penggunaan indikasi seperti pada PRN(prorenataatau“jika perlu”) atau instruksi pengobatanlain;
  5. jenis instruksi pengobatan yang berdasar atas berat badan seperti untuk anak anak, lansia yang rapuh, dan populasi khusus sejenis lainnya;
  6. kecepatan pemberian (jika berupainfus);
  7. instruksi khusus, sebagai contoh: titrasi, tapering, rentangdosis.

Standar ini berlaku untuk resep atau permintaan obat dan instruksi pengobatan di semua unit pelayanan di rumah sakit. Rumah sakit diminta memiliki proses untuk menjamin penulisan resep atau permintaan obat dan instruksi pengobatan sesuai dengan kriteria butir1 sampai dengan 4 diatas.

Untuk memilih dan menentukan obat yang dibutuhkan pasien diperlukan pengetahuan dan pengalaman spesifik.Rumah sakit bertanggung jawab menentukan staf  medis dengan pengalaman cukup dan pengetahuan spesifik sesuai dengan peraturan perundang-undangan yang diberi izin membuat/menulis resep atau membuat permintaan obat.

Rumah sakit membatasi penulisan resep meliputi jenis dan jumlah obat oleh staf medis, misalnya resep obat berbahaya, obat kemoterapi, obat radioaktif, dan obat untuk keperluan investigasi. Staf medis yang kompeten dan diberi kewenangan membuat atau menulis resep harus dikenal dan diketahui oleh unit layanan farmasi atau lainnya yang memberikan atau menyalurkan obat. Dalam situasi darurat maka rumah sakit menentukan tambahan PPA yang diberi izin untuk membuat atau menulis resep atau permintaan obat dan instruksi pengobatan.
