Standar Pelayanan dan Asuhan Pasien (PAP) Akreditasi Rumah Sakit

Dalam standar Pelayanan dan Asuhan Pasien (PAP) disebutkan bahwa tanggung jawab rumah sakit dan staf yang terpenting adalah memberikan asuhan dan pelayanan pasien yang efektif dan aman. Hal ini membutuhkan komunikasi yg efektif, kolaborasi, dan standardisasi proses untuk memastikan bahwa rencana, koordinasi, dan implementasi asuhan mendukung serta merespons setiap kebutuhan unik pasien dan target.
Asuhan tersebut dapat berupa upaya pencegahan, paliatif, kuratif, atau rehabilitatif termasuk anestesia, tindakan bedah, pengobatan, terapi suportif, atau kombinasinya, yang berdasar atas asesmen dan asesmen ulang pasien.
Area asuhan risiko tinggi (termasuk resusitasi, transfusi, transplantasi organ/jaringan) dan asuhan untuk risiko tinggi atau kebutuhan populasi khusus yang membutuhkan perhatian tambahan.
Asuhan pasien dilakukan oleh profesional pemberi asuhan (PPA) dengan banyak disiplin dan staf klinis lain. Semua staf yg terlibat dalam asuhan pasien harus memiliki peran yg jelas, ditentukan oleh kompetensi dan kewenangan, kredensial, sertifikasi, hukum dan regulasi, keterampilan individu, pengetahuan, pengalaman, dan kebijakan rumah sakit ,atau uraian tugas wewenang (UTW).

Beberapa asuhan dapat dilakukan oleh pasien/keluarganya atau pemberi asuhan terlatih (care giver).
Pelaksanaan asuhan dan pelayanan harus dikoordinasikan dan diintegrasikan oleh semua profesional pemberi asuhan (PPA) dapat dibantu oleh staf klinis lainnya.
Asuhan pasien terintegrasi dilaksanakan dengan beberapa elemen.
⦁ Dokter penanggung jawab pelayanan (DPJP) sebagai pimpinan klinis/ketua tim PPA (clinical leader).
⦁ PPA bekerja sebagai tim interdisiplin dengan kolaborasi interprofesional, menggunakan alur klinis/clinical pathway, perencanaan pemulangan pasien terintegrasi/integrated discharge planning.
⦁ Manajer Pelayanan Pasien/Case Manager menjaga kesinambungan pelayanan.
⦁ Keterlibatan serta pemberdayaan pasien dan keluarga dalam asuhan bersama PPA harus memastikan:
⦁ asuhan direncanakan untuk memenuhi kebutuhan pasien yang unik berdasar atas asesmen;
⦁ rencana asuhan diberikan kepada tiap pasien;
⦁ respons pasien terhadap asuhan dimonitor;
⦁ rencana asuhan dimodifikasi bila perlu berdasar atas respons pasien.
