Menentukan Penugasan Staf Medis

Rumah sakit menyelenggarakan pengumpulan dokumen kredensial dari anggota staf medis yang diberi izin memberikan asuhan kepada pasien secara mandiri. 

Rumah sakit melaksanakan verifikasi terkini terhadap pendidikan, registrasi, izin, pengalaman, dan lainnya dalam kredensialing staf medis.

Maksud dan Tujuan

Istilah yang digunakan di standar dijelaskan sebagai berikut:

Kredensial adalah proses evaluasi oleh suatu rumah sakit terhadap seorang staf medis untuk menentukan apakah yang bersangkutan layak diberi penugasan klinis dan kewenangan klinis untuk menjalankan asuhan/tindakan medis tertentu dalam lingkungan rumah sakit tersebut untuk periode tertentu.
 
Dokumen kredensial adalah dokumen yang dikeluarkan oleh badan resmi untuk menunjukkan bukti telah dipenuhinya persyaratan seperti ijazah dari fakultas kedokteran, surat tanda registrasi, izin praktik, fellowship, atau bukti pendidikan dan pelatihan yang telah mendapat pengakuan dari organisasi profesi kedokteran. Dokumen dokumen ini harus diverifikasi dari sumber utama yang mengeluarkan dokumen. Dokumen kredensial dapat juga diperoleh dari rumah sakit, perorangan, badan hukum yang terkait dengan riwayat profesional, atau riwayat kompetensi dari pelamar seperti surat rekomendasi, semua riwayat pekerjaan sebagai staf medis di tempat kerja yang lalu, catatan asuhan klinis yang lalu, riwayat kesehatan, dan foto. 
 
Dokumen ini akan diminta rumah sakit sebagai bagian dari proses kredensial dan ijazah serta STR harus diverifikasi ke sumber utamanya. Syarat untuk verifikasi kredensial disesuaikan dengan posisi pelamar. Sebagai contoh, pelamar untuk kedudukan kepala departemen/unit layanan di rumah sakit dapat diminta verifikasi terkait jabatan dan pengalaman administrasi dimasa lalu. Juga untuk posisi staf medis dirumah sakit dapat diminta verifikasi riwayat pengalaman kerja beberapa tahun yang lalu.
 

Staf medis adalah semua dokter dan dokter gigi yang memberikan layanan promotif, preventif, kuratif, rehabilitatif, bedah, atau layanan medis/gigi lain kepada pasien, atau yang memberikan layanan interpretatif terkait pasien seperti patologi, radiologi, laboratorium, serta memiliki surat tanda registrasi dan surat izin praktik.

Verifikasi adalah sebuah proses untuk memeriksa validitas dan kelengkapan kredensial dari sumber yang mengeluarkan kredensial. Proses dapat dilakukan ke- fakultas/rumah sakit/perhimpunan di dalam maupun di luar negeri melalui email/surat konvensional/pertanyaan on line/atau melalui telepon. Verifikasi dengan email maka alamat email harus sesuai dengan alamat email yang ada pada website resmi universitas/rumahsakit/perhimpunan profesi tersebut dan bila melalui surat konvensional harus dengan pos tercatat.
 
Kredensial adalah sebuah proses memeriksa dokumendari pelamar, wawancara,dan ketentuan lain sesuai dengan kebutuhan rumah sakit untuk memutuskan apakah seorang memenuhi syarat diberi rekomendasi kewenangan klinis untuk memberikan asuhan pasien yang dibutuhkan pasien. Untuk pelamar baru, informasi yang diperiksa terutama berasal dari sumber luar.
