Pemahaman mutu layanan klinis

Standar

4.2.  Mutu layanan klinis dan Keselamatan dipahami dan didefinisikan dengan baik oleh semua pihak yang berkepentingan.

Kriteria

1.2.1.   Fungsi dan proses layanan klinis yang utama diidentifikasi dan diprioritaskan dalam upaya perbaikan mutu layanan klinis dan menjamin keselamatan.

Pokok Pikiran:

  • Dengan adanya keterbatasan sumber daya yang ada di klinik, maka upaya perbaikan mutu layanan klinis perlu diprioritaskan.  Oleh karena itu tenaga klinis bersama dengan pengelola klinik menetapkan prioritas fungsi dan proses pelayanan yang perlu disempurnakan. Penetapan prioritas dilakukan dengan kriteria tertentu misalnya: high risk, high volume, high cost, dan kecenderungan terjadi masalah, atau didasarkan atas penyakit, kelompok sasaran, program prioritas atau pertimbangan lain.

Elemen Penilaian:

  1. Dilakukan identifikasi fungsi dan proses pelayanan yang prioritas untuk diperbaiki  dengan kriteria yang ditetapkan
  2. Terdapat dokumentasi tentang komitmen dan pemahaman terhadap peningkatan mutu dan keselamatan secara berkesinambungan ditingkatkan dalam organisasi
  3. Setiap tenaga klinis dan manajemen memahami pentingnya peningkatan mutu dan keselamatan dalam layanan klinis ,
  4. Pimpinan klinik bersama dengan tenaga klinis menetapkan pelayanan prioritas yang akan diperbaiki.
  5. Pimpinan klinik bersama dengan tenaga klinis menyusun rencana perbaikan pelayanan prioritas yang ditetapkan dengan sasaran yang jelas.
  6. Pimpinan klinik bersama dengan tenaga klinis melaksanakan kegiatan perbaikan pelayanan klinis sesuai dengan rencana
  7. Dilakukan evaluasi terhadap pelaksanaan kegiatan perbaikan pelayanan klinis.

Kriteria

1.2.2.   Ada pembakuan standar  layanan klinis yang disusun berdasarkan acuan yang jelas. 

Pokok Pikiran:

  • Agar pelayanan klinis dapat dikendalikan dengan baik, maka perlu dilakukan pembakuan standar dan prosedur layanan klinis.  Standar dan prosedur tersebut perlu disusun berdasarkan acuan yang jelas dan dapat dipertanggungjawabkan, dan bila memungkinkan berdasarkan bukti ilmiah terkini dan yang terbaik (the best available evidence).

 Elemen Penilaian:

  1. Standar/prosedur layanan klinis disusun dan dibakukan didasarkan atas prioritas fungsi dan proses pelayanan
  2. Standar tersebut disusun berdasarkan acuan yang jelas
  3. Tersedia dokumen yang menjadi acuan dalam penyusunan standar
  4. Ditetapkan prosedur penyusunan standar/prosedur layanan klinis
  5. Penyusunan standar/prosedur layanan klinis sesuai dengan prosedur
