1.    Siapkan laporan insiden yang akan di investigasi
2.    Tentukan tim investigasi
3.    Lakukan Investigasi insiden 
4.    Kumpulkan data, dengan cara:
       a.    Observasi        : Kunjungan langsung
       b.    Dokumentasi    : Apa yang terjadi sesuai data observasi dan inspeksi
        ( foto, foto copy, laporan dll) 
       c.    Interview    : Mengetahui kejadian secara langsung untuk pengecekan pada hasil observasi dan data dokumentasi                        
5.    Petakan informasi kronologis insiden
a.    Kronologis masalah: disiapkan untuk lebih memudahkan analisa
b.    Timeline: rantai insiden, mencari bagian dalam proses dimana masalah terjadi
c.    Tabular Timeline: sama dengan timeline, namun lebih detil, menginformasi kan good practice/SPO,  identifikasi masalah
d.    Time person grid: melacak gerak/kehadiran personel, sebelum dan sesudah insiden
e.    Flowchart        : Serangkaian kejadian secara sederhana
6.    Identifikasi masalah /Care Management Problem /CMP
        Prinsip dasarnya;     
        ⦁    Semua bentuk pelayanan yang menyimpang dari standar pelayanan yang ditetapkan
        ⦁    Penyimpangan yang terjadi, memberikan dampak yang langsung dan tidak langsung terhadap IKP
7.    Analisa informasi
       a.    5 Why: Why why chart, menanyakan setiap penyebab masalah yang teridentifikasi dan untuk mengidentifikasi
       b.    Change analysis    : 
              ⦁    Analisa proses yang tidak bekerja sesuai dengan rencana (what & why change)
              ⦁    Merupakan metode sederhana yang dapat membantu membandingkan proses yang berjalan efektif atau gagal
              ⦁    Apa yang berubah dan dampaknya (potensial & actual)
              ⦁    Digunakan bila; system/tugas yang awalnya berjalan efektif, kemudian terjadi kegagalan/terdapat sesuatu yang menyebabkan perubahan situasi dan mencurigai suatu perubahan tersebut menyebabkan ketidak sesuaian tindakan atau kerusakan alat.
      c.    Barrier analysis
            ⦁    Prospectively/kemungkinan, untuk identifikasi potensial hazards (bahaya), target dan potensial solusi
            ⦁    Reactively, mengikuti insiden untuk  identifikasi penghalang yang harusnya ada untuk mencegah atau memitigasi insiden (tuntutan)
            ⦁    Ada 4 tipe: Physical barrier, Natural barrier, Human Action barrier, Administrative barrier (mis SPO, supervise & training, dll)
      d.    Fishbone
