CONTOH PROSEDUR PERMINTAAN PERSETUJUAN AKSES IT RUMAH SAKIT

    1. Pemohon mengisi Form Permintaan Akses User untuk Fasilitas IT , lengkap dengan alasan kenapa fasilitas itu dibutuhkan untuk pekerjaan sehari-hari.

  1. Pemohon meminta persetujuan dari
    1. Atasan Langsung Pemohon,
    2. Kepala Departemen Pemohon,
    3. Pimpinan Perusahaan sesuai SPO ini.
  2. Kepala Departemen atau Manager IT yang berwenang me-review untuk memastikan kapasitas dan keamanan Infrastructure dan System IT.
  3. Bila ada bagian yang tidak memungkinkan atau membahayakan, Kepala Departemen atau Manager IT akan membicarakannya dengan Pejabat / Pimpinan yang telah menyetujui.
  4. Bila tidak masalah, Form Akses tersebut ditanda-tangani untuk diproses.
  5. Jika semua persetujuan sudah didapat, Petugas IT yang berwenang akan memberikan fasilitas IT Khusus tersebut sesuai Form Akses.
  6. Setelah siap, Departemen IT memberikan informasi kepada Pemohon dan Unit Kerja terkait bahwa permintaan sudah dikerjakan
  7. Pemohon melakukan pengecekan apakah pemberian Fasilitas IT sudah sesuai atau tidak sesuai dengan permintaan.
  8. Jika belum sesuai, Pemohon memberikan informasi ke Departemen IT untuk me-review kembali dan melakukan perubahan sesuai dengan permintaan yang disetujui.
  9. Jika sesuai permintaan dan persetujuan yang didapat, Pemohon menanda-tangani Form Akses di bagian penyelesaian sebagai bukti permintaan telah dipenuhi.
