Pelayanan kefarmasian dan penggunaan obat (PKPO)

Pelayanan kefarmasian adalah pelayanan langsung dan bertanggung jawab kepada pasien yang berkaitan dengan sediaan farmasi dan alat kesehatan dengan maksud mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien.
Pelayanan kefarmasian di rumah sakit bertujuan untuk
⦁ menjamin mutu, manfaat, keamanan, serta khasiat sediaan farmasi dan alat kesehatan;
⦁ menjamin kepastian hukum bagi tenaga kefarmasian;
⦁ melindungi pasien, masyarakat, dan staf dari penggunaan obat yang tidak rasional dalam rangka keselamatan pasien (patient safety);
⦁ menjamin sistem pelayanan kefarmasian dan penggunaan obat yang lebih aman (medication safety);
⦁ menurunkan angka kesalahan penggunaan obat.

Pelayanan kefarmasian dan penggunaan obat merupakan komponen yang penting dalam pengobatan simtomatik, preventif, kuratif, paliatif, dan rehabilitatif terhadap penyakit dan berbagai kondisi, serta mencakup sistem dan proses yang digunakan rumah sakit dalam memberikan farmakoterapi kepada pasien. Pelayanan kefarmasian dilakukan secara multidisiplin dalam koordinasi para staf di rumah sakit.

Rumah sakit menerapkan prinsip rancang proses yang efektif, implementasi dan peningkatan mutu terhadap seleksi, pengadaan, penyimpanan, peresepan atau permintaan obat atau instruksi pengobatan, penyalinan (transcribe), pendistribusian, penyiapan (dispensing), pemberian, pendokumentasian, dan pemantauan terapi obat. Praktik penggunaan obat yang tidak aman (unsafe medication practices) dan kesalahan penggunaan obat (medication errors) adalah penyebab utama cedera dan bahaya yang dapat dihindari dalam sistem pelayanan kesehatan di seluruh dunia. Oleh karena itu, rumah sakit diminta untuk mematuhi peraturan perundang-undangan, membuat sistem pelayanan kefarmasian, dan penggunaan obat yang lebih aman yang senantiasa berupaya menurunkan kesalahan pemberian obat.
