1.    Dokter mengisi formulir cuti yang tersedia di Poliklinik, dengan isi sebagai berikut :

       d.    Tanggal Cuti
       e.    Alasan Pengambilan Cuti
       f.     Dokter Pengganti
       g.    Tanda Tangan Dokter yang Cuti dan tanda tangan dokter pengganti
2.    Dokter menyerahkan formulir yang sudah diisi lengkap ke perawat yang selanjutnya diserahkan ke bagian pendaftaran.

Sumber Data dalam Melakukan Monitoring Staf Klinis

Rumah sakit mengumpulkan berbagai data untuk keperluan manajemen, misalnya membuat laporan ke pimpinan rumah sakit tentang alokasi sumber daya atau sistem pembiayaan rumah sakit. Agar bermanfaat bagi evaluasi berkelanjutan seorang staf medis maka sumber data rumah sakit harus dikumpulkan sedemikian rupa agar teridentifikasi staf medis yang berperan. Harus terkait dengan praktik klinis seorang anggota staf medis; dapat menjadi rujukan(benchmark) didalam KSM/Unit layanan atau diluarnya untuk mengetahui pola individu staf medis.
