Contoh Prosedur Audit Rekam Medik

  1. Pengumpulan data akan dilakukan pada Hari Selasa dan Jum’at secara proposional dengan acak sederhana.
  2. Menentukan jumlah sample dari jumlah kunjungan bulan lalu.
  3. Mengelompokan jenis medical record berdasarkan jenis kelengkapan seperti : Emergency, OPD, Ward, Invasif, Ward Non Invasif
  4. Untuk jenis kelengkapan pada OPD / MC, petugas yang berwenang akan membuka system akan melihat seluruh nomor yang ada dan diambil secara acak yang akan diserahkan kepada petugas yang berwenang yang menangani masalah tersebut.
  5. Untuk kelengkapan pada medical record Emergency, Ward Invasif, Non Invasif , petugas yang berwenang mengambil file medical record ditempat penyimpanan file medical record yang diambil secara acak dan akan diserahkan kepada petugas yang berwenang menangani masalah tersebut.
  6. Hasil rekapan tersebut akan ditandatangin oleh Kepala Unit Medical Record dalam 1 minggu dan diserahkan kepada Kepala sub komite rekam medik.

  7. Sub komite rekam medik akan memeriksa hasil audit rekam medik :

  8. Bila tidak ada masalah 2 bulan sekali dengan jumlah 25  Medical Record setiap jenis.
  9. Bila diketemukan permasalahan seperti penyimpangan hasil, pengulangan kesalahan lebih dari 2 kali pemeriksaan dalam segi diagnosis dan hasil.
  10. Kekurangan yang berkaitan dengan masalah uokum maka audit akan dilakukan dalam waktu maksimal 3 x 24 jam dengan mengambil contoh salah satunya dari hasil rekap sementara.
  11. Hasil audit akan dilaporkan kepada : Leader MR, Sub Komite Rekam Medik, COO, Medical Manager.
